PRN Nurse Practitioner in Lafayette, LA

Homedica is seeking a compassionate and skilled PRN Nurse Practitioner to deliver comprehensive primary care services across a variety of settings-including in-home visits, assisted living and nursing facilities, post-acute care hospitals, and via telehealth.

Responsibilities:

• Provide primary medical care services in home, facility, and telehealth settings
• Conduct physical exams, wellness visits, and manage chronic and acute conditions
• Order and interpret diagnostic tests and labs
• Prescribe and manage medications and individualized care plans
• Coordinate care with interdisciplinary teams and community partners
• Promote preventive care, patient education, and continuity of care
• Participate in after-hours or on-call rotation as needed

Qualifications:

• Active Nurse Practitioner license in Louisiana
• DEA license required
• Experience in home health, hospice, or facility-based care required
• Strong communication, decision-making, and clinical skills
• Comfortable working independently across diverse care environments
