The Behavioral Health Clinician I provides care to individuals with disabilities in the designated single “program units” of the CCH housing facilities and / or in off-site units. The goal is to assist these individuals in maintaining their housing and to provide the services, linkages, and clinical interventions necessary to prevent them from returning to homelessness and to live as independently as possible. This position will be responsible for the intake, assessment, treatment planning, and service provision to adult individuals with disabilities.
