Responsibilities
Located in downtown Fresno, Community Regional is home to the Central Valley’s only Level I Trauma Center and one of California’s most advanced cardiac programs — caring for a broad range of high-acuity and complex cardiac patients. As an RN Navigator for Cardiac Services,you’llserve as a consistent point of connection for patients and families, helping them understand their care plan and move confidently through the cardiac continuum while partnering closely with multidisciplinary teams across the hospital.
- Serve as the primary clinical resource and liaison for assigned cardiac patients, ensuring coordinated care from referral through post-procedure follow-up.
- Partner with cardiologists, cardiac surgeons, interventionalteamsand inpatient units to streamline care pathways and enhance communication.
- Monitor patient progress,identifyneedsand intervene proactively to support safety,continuityand adherence toevidence-based guidelines.
- Contribute to performance improvement, patient-experienceinitiativesand multidisciplinary review processes within the cardiac service line.

Qualifications
- High School Diploma, High School Equivalency (HSE)or Completion of a CHS Approved Individualized Education Plan (IEP) Certificaterequired
- 3 years of recent nursing experience in Cardiology/Pulmonology and/or Cath Lab/Care Coordinationrequired
- RN - Current and valid Registered Nurse license to work within the state of Californiarequired
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)required
- NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degreerequired
