Oregon Licensed Practical Nurse

A company is looking for a Remote Licensed Practical Nurse (LPN) with Oregon Licensure for a day shift position.

Key Responsibilities :

Manage patient census to close gaps in clinical and non-clinical patient care

Identify and coordinate community resources for patient care

Provide patient education on managing chronic conditions and perform medication management

Required Qualifications :

Active Oregon License as a Licensed Practical Nurse (LPN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a sedentary position and communicate effectively by telephone

Must have a home office or HIPAA-compliant workspace
