Description :

The position involves case management responsibilities, working 40 hours a week, Monday to Friday.

Candidates should have strong inpatient hospital case management experience.

Experience in an acute care setting and discharge planning is necessary.

Familiarity with Medicare, Medicaid, managed care, and other third-party payer review requirements is essential.

The role ensures the medical necessity of hospitalization and treatment in accordance with guidelines and criteria.

The position requires appropriate utilization of hospital resources.

Collaboration with other physicians and healthcare team members to manage care efficiently and cost-effectively is a key responsibility.

The individual is responsible for the integration, coordination, and advocacy for patients and families requiring post-hospital services.

Achieving planned care outcomes by planning and arranging services upon discharge is expected.

The case manager acts as a liaison with medical staff, other departments, patients, families, and community resources to facilitate the utilization management review and discharge planning process.
