We are seeking a compassionate and knowledgeable Dietitian or Dietetic Technician, Registered (DTR) to support the nutritional needs of residents across our skilled nursing, rehabilitation, and assisted living communities in the Cleveland area. The ideal candidate will bring strong clinical skills, excellent communication, and a commitment to enhancing resident health and quality of life. Complete nutritional assessments and reassessments for residents in skilled nursing, rehab, and assisted living settings.
Develop individualized nutrition care plans based on clinical needs, physician orders, and resident preferences.
Monitor weight changes, intake, and overall nutritional status, intervening as needed.
Participate in interdisciplinary team meetings and contribute to resident care plans.
Communicate effectively with nursing, therapy, dietary staff, and physicians.
Provide nutrition education to residents, families, and staff as appropriate.
Maintain accurate and timely documentation in accordance with state, federal, and facility standards.
Ensure compliance with CMS, DOH, and other regulatory requirements.
Support survey readiness and quality improvement initiatives.
Assist with menu planning and review to ensure nutritional adequacy and resident satisfaction.
Collaborate with dietary staff to support safe food service practices and special diet accommodations.
Promote a positive dining experience that enhances resident wellbeing.
Registered Dietitian (RD/RDN) or Dietetic Technician, Registered (DTR) required.
Previous healthcare experience required (skilled nursing, rehab, or assisted living preferred).
Strong understanding of clinical nutrition, therapeutic diets, and regulatory standards.
Excellent communication, organization, and teamwork skills.
