Job Title: Home Health Occupational TherapistJob Responsibilities:

• Conduct initial assessments/re-assessments and develop individualized care plans for clients
• Provide skilled occupational therapy to clients in their homes, including therapeutic interventions and goals, home exercise program, etc.
• Coordinate with other members of the care team to ensure comprehensive and integrated care for clients
• Educate clients and their families on proper self-care techniques and health maintenance strategies
• Monitor and document client progress and communicate any changes or concerns to the physician
• Act as an advocate for clients and ensure their needs are met in a timely and respectful manner
• Adhere to all company policies, procedures, and regulatory requirements

Job Qualifications:

• Active OT license in the state of Nebraska
• Minimum of 2 years of occupational therapy experience, preferably in home health or a similar setting
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively as part of a team
• Compassionate and patient-centered approach to care
