[job_card.job_description]Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care. Physical Therapist Professional license in Massachusetts required. 1 Year Home Care experience required. Weekend rotations are once every 6 weeks. Ability to travel to cases as assigned. MGB Badge Photo required for onboarding. MGB MVR Check required within 45 days of start. License / Certificate Verification required within 30 days of start date; MA Conditional Licenses not accepted. N95 Fit Test required within 365 days of start date and updated annually. MGB Nurse Aide Registry check required within 30 days of start. Documentation of MMR vaccination or immunity required. Influenza Vaccine required during flu season (Sept 15 - April 30). TB Test documentation required within 3 months of start date and updated if > Health Statement signed by MD / DO / NP / PA required within 365 days of start date and updated if > MGB Criminal Background Check required within 45 days prior to start date and updated if > MGB Massachusetts CORI required within 30 days prior to start date and updated if > MGB Resume including past 7 years of work experience required for submission. Respirator Medical Clearance Attestation required if fit test is required. MGB References (2 recent, minimum 1 supervisor) required for submission. MGB Skills Checklist required for submission. MGB Confirmation of Assignment / POI Form required for all initial assignments and extensions. MGB Education Verification required for onboarding. MGB SSN Trace required for onboarding. MGB National Sex Offender Check required within 30 days of start and updated annually. MGB Exclusion Checks required within 30 days of start and annually. MGB Candidate Submission Coversheet required for submission.
