Licensed Clinical Social Worker (LCSW)



As a Licensed Clinical Social Worker (LCSW), you'll play a key role in supporting patients’ mental and emotional well-being as part of an integrated care team. This position involves direct patient care, counseling, and coordination with both internal teams and external resources to provide comprehensive support across a variety of medical and behavioral health needs.

What You’ll Do

  • Conduct mental health assessments, brief interventions, and integrated care consultations for patients across all age groups

  • Provide individual and family counseling tailored to a variety of needs and backgrounds

  • Educate patients and families on behavioral health topics, treatment options, and available support services

  • Collaborate closely with medical and behavioral health professionals to ensure coordinated, patient-centered care

  • Refer patients to appropriate community resources and follow up to support continuity of care

  • Maintain timely and accurate documentation, including assessments, treatment plans, and progress notes

  • Participate in team meetings, trainings, and ongoing professional development

  • Contribute to the development or improvement of behavioral health programs as needed

What You’ll Need

  • Active LCSW license

  • Experience providing clinical care in a healthcare or integrated care setting

  • Strong communication and collaboration skills

  • Familiarity with community mental health and substance abuse resources

  • Comfort working with diverse populations and age groups

  • Proficiency with electronic health records and clinical documentation
