Key Responsibilities

• Lead and oversee all aspects of patient care operations in a home health setting, ensuring delivery of exceptional clinical services
• Ensure strict compliance with Medicare Conditions of Participation, state licensing regulations, and all federal, state, and local healthcare requirements
• Supervise, mentor, and conduct performance evaluations for nursing staff, fostering professional growth and development
• Collaborate with interdisciplinary healthcare teams to develop and implement personalized, patient-centered care plans
• Monitor and ensure timely, accurate clinical documentation and maintain comprehensive patient health records
• Conduct regular audits of patient care records to ensure quality assurance standards and regulatory compliance
• Facilitate effective communication among patients, families, physicians, and healthcare team members for seamless care coordination
• Lead quality improvement initiatives and implement evidence-based best practices to enhance patient outcomes
• Participate in ongoing professional development and stay current with healthcare industry trends and regulations
• Manage patient admissions, care transitions, and discharge planning processes

Required Qualifications

• Current, active Registered Nurse (RN) license in the state of Pennsylvania
• Minimum of two (2) years of management or supervisory experience in a healthcare setting, preferably in home health or home care
• Previous experience working in a home health care or home care environment strongly preferred
• Comprehensive knowledge of accepted professional nursing standards and clinical best practices
• Thorough understanding of Medicare Conditions of Participation (CoPs) and OASIS documentation requirements
• Strong knowledge of federal, state, and local healthcare regulatory requirements and compliance standards
• Excellent leadership, team management, and staff development capabilities
• Outstanding interpersonal and communication skills with patients, families, and healthcare professionals
• Proven ability to develop and implement quality improvement initiatives
• Strong organizational skills with attention to detail and ability to manage multiple priorities
• Proficiency with electronic health records (EHR) and healthcare documentation systems
• Bachelor of Science in Nursing (BSN) preferred
