Role Snapshot

• Nurse Practitioner
• $140/visit
• Metro OR (Washington + Clackamas Counties)
• In-home, local travel

What you'll do

• Complete 6–9 in-home health assessments/day

• Review medical history and risk factors

• Collect vitals, labs, and screening data

• Document findings in EMR

• Educate members on next steps

• Coordinate follow-up if red flags show

• Travel across Washington and Clackamas Counties only

Must-haves

• Active NP license

• Master's in Nursing or higher

• Valid driver's license + reliable car

• Able to work independently in the field
