Registered Nurse - Clinical Care Guide/Case Manager

We are seeking a compassionate, highly organized Registered Nurse Clinical CareGuide to join our Advanced Primary Care team in Scottsdale, AZ. In thispatient-centered role, you will provide end-to-end support for specialist anddiagnostic testing navigation, ensuring patients receive seamless, coordinatedcare throughout their health journey. As a key clinical partner, you willcollaborate closely with providers, care coordinators, and support staff tohelp patients achieve their best possible health outcomes.

Key Responsibilities:

• Serve as the main point ofcontact for specialist and diagnostic testing navigation, including schedulingand follow-up.
• Support patients throughindividualized care plans and promote proactive health management.
• Conduct clinical assessmentsand provide guidance aligned with the Advanced Primary Care model.
• Assist with caretransitions, including post-discharge follow-up and coordination with communityresources.
• Maintain accurate electronicmedical documentation.
• Provide excellent customerservice and build strong patient relationships.
• Perform other duties asassigned.
• Work flexible hours,including mornings, evenings, or weekends.
• Travel between care centersas needed; position is 100% on-site.

Requirements:

• Current Arizona RegisteredNurse license.
• BLS certification required.
• Experience with ElectronicMedical Records (EMR), including Medicare-based systems.
• Strong customer serviceorientation with experience in patient-facing roles.
• Excellent verbal, written,and electronic communication skills.
• Active AZ FingerprintClearance Card required.
