Job Details

  • Night shift works every other weekend, every other holiday
  • BLS flexibility with scheduling a MUST - no block schedules

    • Job Requirements

  • Driver's License / State ID required for onboarding
  • Prime OIG required for onboarding
  • Prime Drug Screening (minimum 10-panel test) within 60 days of start
  • Prime Color-blind test, updated annually
  • Prime Agency Attestation completed after all other credentials uploaded
  • Prime SAM / EPLS search for 'Excluded Individual' required
  • Up-to-date resume required at offer acceptance
  • Two references required, one from a manager or direct supervisor
  • Name Verification Affidavit if name discrepancies between credentials
  • Primary Source Verification of professional license within 30 days of start
  • COVID-19 Vaccine : 2 vaccines and booster required or declination completed
  • Proof of MMR immunity (2 vaccines or positive titer)
  • Annual Respirator Fit Test required prior to start
  • Hepatitis B vaccination required (declinations not accepted at some locations)
  • Influenza Vaccine or declination required during flu season
  • TB Test within one year of start date
  • Varicella vaccination or immunity proof required
  • Tdap vaccination required (declinations not accepted at some locations)
  • Annual TB Questionnaire required
  • Positive PPD with Negative Chest X-Ray optional
  • Social Security Card or Passport required
  • Physical / Health Statement annually, must state clear to work
  • Prime Core Competencies Exam with minimum 80% score
  • Prime Skills Checklist required
  • National Criminal Background Check within one year of start date
  • Prime Badge Photo (JPEG) required
  • Prime Form I-9 (Employment eligibility verification) required
  • Prime Job Description signed by employee
  • Essentials Onboarding Packet required
  • Prime Orientation Packet required
  • Onboarding Education Attestation required
