Job Summary:
The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential. This position is for a 1-year contract starting in February, working in a Step Down unit.

Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Monitor and report changes in patient conditions.
- Document patient care and related duties.
- Float between units as needed.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Minimum two years of hospital, home health, or hospice care experience.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work.

Special Considerations:
- Must wear solid navy blue colored uniforms or scrub attire (scrub tops may have colored piping).
- Weekends are required, every other weekend.
- Will float between units.

Scheduling:

- Block schedules are not required.
- Ratios: 1:4
- 48-hour work weeks are approved.
