Nurse Practitioner - Primary Care physician job in Virginia :

seeking a Primary Care Nurse Practitioner for new ESKD Program

Our client is pioneering a new approach to primary care for patients with end-stage renal disease (ESKD) directly within their dialysis centers.

Monday-Friday role

Schedule will revolve around the time patients will come into the dialysis units, primarily between 8 : 00 AM and 5 : 00 PM.

Work from a portfolio of 3-8 dialysis centers

You will have clinical autonomy to determine which patients to see and during which time.

Average 10-12 patient visits per day.

Proximity to and coordination with other critical caretakers, such as the Nephrologist, dialysis teammates, and integrated care leaders.

Coordinate patient follow-ups to proactively address patient concerns, including coordinating care with other providers, ordering key labs, and prescribing medication.

Build clinical relationships with nephrologists, dialysis teammates, integrated care teammates, and local PCPs and specialists.

Conduct annual comprehensive health evaluations.

Required : Minimum of 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology, NP license to practice autonomously, VA DEA, and comfort in prescribing for chronic diseases typically associated with ESRD, such as congested heart failure

Preferred : Experience with dialysis or nephrology
