Position : Registered Nurse / Licensed Vocational Nurse

The field nurse will provide direct patient care in the patients place of residence. He / she must be able to function independently in performing the nursing process. Assess patients’ condition during every visit and chart observations. Perform evaluation tasks, including vital signs and medication review and implement the plan of care provided by the clinician. Submission of timely documentation via our e-charting system, Kantime, is required.

Duties

• Provides direct, hands-on care to assigned clients.
• Observes and assesses the client’s condition and care needs appropriately and reports changes in condition to the supervisor and / or physician, as appropriate.
• Administers prescribed medications, treatments, and therapies.
• Medication Monitoring
• Takes appropriate nursing action based on current assessments and achieves expected outcomes.
• Educates and provides ongoing support and education to families and caregivers.
