We are looking for an experienced, dynamic RN Case Manager to join our team.

Position Summary

As a  Case Manager , you will be responsible for the coordination of care with the patient, family, and other members of the team. Our Case Managers work as RN / LPN teams so we need someone with great supervisory and organization skills.

Knowledge, Skills and Abilities

• Must be comfortable with driving.
• Must be comfortable using EMR
• Must have an energetic and positive demeanor
• Must be able to remain calm under patient pressure.
