Registered Nurse Quality Care Manager

A company is looking for a Quality Care Manager (QCM) Home Health / Registered Nurse (RN).

Key Responsibilities :

Communicates effectively with the healthcare team and collaborates with leadership and other departments

Reviews documentation and plans of care for congruency and compliance with agency policies and regulations

Approves and ensures accuracy of the final Plan of Care and reviews OASIS assessments for inconsistencies

Qualifications :

Graduate of an accredited School of Nursing

Current unencumbered license to practice as a Registered Nurse (RN)

Minimum of 1 year of home health experience with knowledge of Medicare Home Health Conditions of Participation and OASIS

Strong computer skills with knowledge of Home Care Home Base preferred

Self-starter with the ability to work independently and manage stress effectively
