Role and Responsibilities

Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse.

Essential Functions

Under the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse :

• Provide health education according to the teaching plan established by the RN.
• Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
• Observe signs and symptoms and and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager.
• Document any services rendered to the patient, changes noted in the patient’s condition, and / or family and home situations.
• Document communication with RN Case Manager and physician.
• Other duties as assigned.
