This is a full-time opportunity working weekdays from 8 AM to 5 PM, Monday to Friday, with limited after-hours on-call rotation. You will care for patients in Kalamazoo, Calhoun, and / or St. Joseph counties.

Key Responsibilities of the Hospice RN / Care Manager :
- Serve as the primary point of contact for patients and families, guiding them through each step of their hospice journey.
- Deliver and document skilled, hands-on nursing care tailored to each patient's individualized Plan of Care, whether at home, in assisted living, or another facility.
- Conduct initial and ongoing assessments to track patient conditions and adjust care as necessary.
- Collaborate with a multidisciplinary team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create and refine personalized care plans.
- Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and bereavement guidance.
- Educate caregivers on how to safely and confidently care for their loved ones, providing both practical training and emotional reassurance.
- Communicate significant updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
- Maintain accurate, current clinical documentation and adhere to infection control and compliance standards.
- Support the organization’s quality improvement efforts and scheduling initiatives to ensure care remains compassionate and efficient.
- Assist patients and families in understanding and navigating essential topics such as medication administration, hospice philosophy and services, symptom and pain management, and the end-of-life process.
Qualifications - What You'll Bring :
Active RN license in the state you are practicing (or eligible to obtain).1+ years of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.A compassionate heart for hospice care : empathetic and patient-centric.Capable of providing care in diverse settings, including private homes and facilities.Strong communication and critical thinking skills, especially in emotionally-sensitive situations.Valid driver’s license, auto insurance, and reliable transportation are required.CPR certification is mandatory.Preferred Experience (Not Required) :
Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac).Admissions RN experience, wound care, float pool, or travel nursing.Previous experience working with terminally ill patients or serious illness support.Benefits for All Associates (Full-Time, Part-Time & Per Diem) :
