About the Role
Sterling Health Solutions is seeking a highly organized, personable, and dependable Patient Care Coordination & Scheduling Specialist to join our growing Transitional Care Management team. This role works closely alongside our Chief Operating Officer (COO) and plays a critical part in ensuring smooth patient care coordination, efficient provider scheduling, and exceptional communication across patients, clinicians, and internal leadership.

Key Responsibilities
● Coordinate patient care activities to ensure timely and appropriate services ● Schedule patient visits with clinicians and healthcare providers
● Manage and maintain provider calendars, adjusting schedules as needed
● Serve as a primary point of contact for patients, ensuring a positive and supportive experience
● Communicate clearly and professionally with patients, providers, and internal teams
● Support the COO with operational coordination related to patient flow and scheduling ● Maintain accurate scheduling records and documentation
● Respond promptly and professionally to patient and provider inquiries

Preferred Qualifications
● Experience with Transitional Care Management, Home Health, or healthcare operations
● Experience in healthcare, patient coordination, or scheduling
● Familiarity with provider scheduling systems or electronic health records (EHRs)
