[job_card.job_description]Position : Radiation Therapist Experience with Truebeam linear accelerator required Work hours : 8am to 4 : 30pm American Heart Association BLS certification required for all direct patient care personnel License / Certificate verification must be dated within 30 days of start Fit Test required Trinity 10 Panel Drug Screen required within 30 days of start Trinity Specialty Specific Competency Testing must be completed within a year Trinity Code of Conduct Form required Trinity I9 form required Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement required Trinity Integrity and Audit Services Form required Trinity Non Employee Upload Form required Trinity Profile including resume, skills checklist within a year, and 2 professional references required Trinity Conflict Of Interest Disclosure Form required Trinity Criminal Background Check including federal, nationwide, county criminal from last 7 years and SSN verification required within 30 days of start Trinity Education Verification required Trinity EPLS report required within past 30 days Trinity GSA / OIG report required within past 30 days Trinity Joint Commission Testing (CORE Exam) within a year required Trinity NSO report required within 30 days of start Trinity System Access Form must be completed and uploaded one week before start Trinity Skills Checklist required for clinical contractors Tdap vaccine or declination accepted if no pediatric interaction Trinity Child and Adult Dependent Abuse Training (Iowa only) Iowa Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start Trinity OFAC check must be completed within 30 days of start Shift : 8 : 00 AM to 4 : 30 PM
