We are seeking an experienced and dedicated Patient Care Manager to join our team in Norwood. This role is critical in ensuring the highest standards of patient care within a home health setting. Key Responsibilities:

  • Manage and oversee patient care services, ensuring compliance with all regulatory standards.

Required Qualifications:

  • Strong knowledge of professional standards, Medicare Conditions of Participation, and compliance with federal, state, and local regulations.

Required Certifications and Licensures:

  • Current licensure as a registered nurse in the state of practice.
