TITLE: 504 CASE MANAGER & SUPPORT
QUALIFICATIONS & REQUIREMENTS:
- Bachelor's degree in education, special education, school psychology, social work, counseling, or a related field
- Valid Illinois Professional Educator License (PEL) or other appropriate licensure/certification relevant to the role
- Knowledge of Section 504 regulations, special education processes, and student support systems
- Experience working with students with disabilities and diverse learning needs
- Strong collaboration, communication, and organizational skills
Preferred:
- Experience serving as a Section 504 Case Manager, school psychologist, or similar role
- Experience providing professional learning or staff training
- Knowledge of behavioral interventions and student mental health supports in school settings
POSITION OVERVIEW:
The 504 Case Manager & Support is a dual-role position that supports equitable access to learning for students with diverse learning needs. The position serves as the primary Section 504 Case Manager for assigned middle schools and provides districtwide consultation and support for Section 504 implementation across other grade levels. In addition, the role supports professional learning, problem-solving, and technical assistance related to Section 504, special education, behavioral supports, and student mental health needs. This position operates under the direction of Student Services leadership and collaborates closely with building and district administrators.
REPORTS TO: Executive Director of Student Support Services
ESSENTIAL DUTIES & RESPONSBILITIES:
Section 504 Case Management (Middle School Focus)
- Serve as the primary Section 504 Case Manager for assigned middle schools
- Coordinate and facilitate Section 504 eligibility meetings, annual reviews, and reevaluations
- Develop, implement, and monitor Section 504 Plans in collaboration with school teams, families, and outside providers
- Ensure compliance with federal and state regulations related to Section 504
- Maintain accurate and timely documentation and records in accordance with district procedures
- Consult with administrators, teachers, and support staff regarding accommodations and implementation of 504 Plans
- Serve as a liaison between families, school staff, and district leadership regarding Section 504 matters
District-Wide 504 Support and Consultation
- Serve as a primary point of contact for Section 504 support and consultation (non-case management) at elementary schools and the high school
- Provide guidance to school teams on Section 504 processes, best practices, and compliance
- Support consistent implementation of Section 504 procedures across grade levels and buildings
- Assist administrators and teams with problem-solving related to student access, accommodations, and supports
Professional Learning and Specialized Supports
- Design and deliver professional learning, training, and consultation for staff related to:Section 504 and Special Education processesBehavioral supports and interventionsStudent mental health and social-emotional supports
- Collaborate with Student Services and other district departments to align practices and supports
- Support the development of inclusive practices that promote student access, equity, and success
- Stay current with applicable laws, regulations, and best practices in special education, Section 504, and student services
Collaboration & Professional Responsibilities
- Collaborate with building and district administrators, teachers, and support staff to strengthen student services systems
- Communicate professionally and effectively with families and community partners
- Participate in required meetings, professional development, and district initiatives
- Maintain confidentiality and demonstrate professionalism and sound judgment in all interactions
- Perform other duties as assigned by administration, consistent with the scope of the role

- Requires collaboration with staff, families, and community partners
- May involve managing multiple priorities across district and building assignments
