With each patient encounter,the incumbent shall use AIDET, perform hand hygiene, assists with positioning patient for comfort and painmanagement, and performs environment check prior to leaving patient s room and / or maintains a clean andorganized environment.Key Responsibilities : Conduct a Physical Therapy evaluation or screen. Develop an individualized Plan of Care with functional goals. Provide appropriate therapeutic interventions. Evaluate patient response to treatment and modify plan as necessary Document the course of patient care including progress made and the continuing need for treatment. Provide patient and / or family education to help achieve goals. Adheres to KHI Attendance Policy. Communicates and demonstrates a professional image for patients, families, clients, coworkers, andothers. Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, andMedicare. Assists with orientation of new staff as requested. Meets KHI requirements for skills competency and safety. Participate in continuing education / professional development activities. Assists with the Physical Therapy and Physical Therapy Assistant student program. Participate in staff meetings, client relation and marketing activities. Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy,and Medicare regulatory requirements. Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements,and is justified in the documentation. Individual productivity meets department standards. Patient satisfaction will meet or exceed department standards.Qualifications :

• Education : Graduate of an APTA approved four year bachelor s degree program, or foreign equivalent.
• Licensure : Current PT License through the AZ Board of Physical Therapy
• Certification : BLS and AED through the American Heart Association
• Experience : Recent experience working with an adult and geriatric populationPreferences :
• Education / experience serving people with physical disabilities is desirable.Work Requirements :
• The Incumbent shall have the ability to lift and transfer patients and equipment, have the ability to stand,walk, sit, bend, stoop, crouch, and communicate to provide therapeutic interventions.Additional Information : If your candidate does not have the license in hand when applying, their tentative start date will be set 4 weeks out in order to allow time for licensing.
