Overview

Team 6 covers Southern Suffolk; Bayport, Bellport, Blue Point, Brookhaven, Farmingville, Holbrook, Holtsville, Lake Ronkonkoma, Patchogue, and Ronkonkoma and the surrounding areas.

  • 8 hour shifts (8 : 30AM to 4 : 30PM) - Rotating Weekends
  • Per visits are required to attend an entire first week of orientation Monday - Friday (8 : 30AM -4 : 30PM.)
  • Per Visits are required to work 1 shift per pay period.

Job Details

Our Home Care PTs :

  • Review patient charts and medical history
  • Develop a personalized treatment plan
  • Teach therapeutic exercises and stretches to improve patient's conditions
  • Use hands-on therapy to promote healing
  • Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
  • Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
  • Provide patient and family education for health promotion as directed by physician's plan of treatment.
  • Provide care to the most upmost standard

    • Minimum Qualifications :

  • New York State PT license
  • Minimum of one year of recent hands on experience as a Physical Therapist.
  • Valid New York State Driver's License and reliable transportation

    • Posted Salary Range

    USD $35.00 - USD $135.00 / Yr.

    This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses / incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
