POSITION SUMMARY
Performs evaluations, planning, and implementation of occupational therapy treatments for inpatients and outpatients. Assists with department goal setting, program planning, clinical instructions and staff instructions in order to maintain a progressive and educationally motivated staff in accordance with the philosophy, goals and objectives of Pender Community Hospital.
ESSENTIAL JOB DUTIES & RESPONSIBILITIES
- Provides individual treatments for activities of daily living (ADL).
- Evaluates patient treatment goals for needed changes according to patient condition.
- Develops treatment plan in collaboration with patient, family support system and members of health care team.
- Completes treatment plan to meet regulatory guidelines.
- Incorporates plans for discharge in treatment plan in collaboration with patient and support system.
- Educates family and / or support system to functional needs, equipment / resources to meet these needs.
- Assures the safety and efficacy of equipment used to provide rehab services.
- Documents assessment, treatment goals, and plan of care according to regulatory guidelines.
- Attends and participates in family conferences, patient care conferences, medical rounds and as required by areas of services to establish goals, formulate discharge summaries and follow-up plans, including home programs.
- Participates in setting and meeting departmental goals by participation in performance improvement projects, on-going program planning and implementation.
- Incorporates patient and support system into treatment and discharge plan.
- Provides information to health care team, payors and surveyors while ensuring confidentiality is maintained.
- Gives direction to rehabilitation assistant for workflow.
- Assumes responsibility for professional growth by participating in continuing education, conferences and in-services to provide progressive therapy.
- Seeks ways to meet patient therapy needs in a cost-effective quality manner.
- Maintains equipment needs and condition of equipment to insure proper department functionality.
- Performs other duties as assigned.
Education, Experience :
Graduate of an accredited Occupational Therapy program.One (1) to three (3) years prior experience preferredCertificates, Licensure, Registrations :
National Board of Certification in Occupational Therapy registered or eligible to take examActive Occupation Therapist licenseActive Physical Agent Modality License.Valid Driver's LicenseBasic Life Support (BLS)GENERAL REQUIREMENTS
