We are a compassionate and growing home health care agency seeking a Licensed Physical Therapist to provide quality, one-on-one therapy services to patients in their homes. This is your chance to work independently while being part of a supportive and experienced team that values excellence in care.
Location: Greater Grand Rapids area
Schedule: Flexible – Full-time or Part-time opportunities available
Responsibilities:
Provide skilled physical therapy services to patients in their homes
Create individualized care plans based on functional goals
Communicate with physicians, patients, and caregivers to ensure coordinated care
Document visits and progress using EMR systems
Requirements:
Current State PT License
At least 1 year of clinical PT experience preferred
Home health experience a plus (but we provide training!)
CPR BLS
Valid driver’s license and reliable transportation
