Registered Nurse – Home Health / Hospice
Location : Charlotte, NC
Shift : 8 hours
Contract Length : 13 weeks
Guaranteed Hours : 40 / week
Position Summary
The Registered Nurse (RN) provides skilled nursing care to home health and hospice patients of all ages in their place of residence. The RN coordinates care with the interdisciplinary team, patient / family, and referring agencies, and is responsible for ongoing care planning, assessment, and clinical oversight.
Key Responsibilities
- Assess home health and hospice patients in stable and emergent situations to determine physical, psychosocial, and environmental needs.
- Develop, implement, evaluate, and document the Plan of Care (POC) to ensure quality and continuity of care.
- Incorporate physical, psychological, social, spiritual, and cultural factors into individualized care plans.
- Maintain accurate and up-to-date patient records, documenting all visits, changes, and interventions.
- Utilize infection control measures to protect patients and staff (OSHA).
- Educate patients and families on care, medications, safety, diet, and self-care techniques.
- Provide clinical supervision and direction to HHAs / STNAs / LVNs.
- Assess and manage pain using appropriate techniques; educate patients / families on pain management.
- Arrange referrals to outside agencies as needed.
- Monitor cases for compliance with third-party payer requirements.
- Participate in on-call rotation and provide on-call patient support.
- Communicate effectively with patients, families, physicians, and interdisciplinary team members.
- Attend team meetings and perform other duties as assigned.
Knowledge, Skills & Abilities
Strong assessment skills and ability to respond to varied patient needs.Effective communication with patients, families, and all organizational levels.Knowledge of medications, venipuncture, IV care, and wound care.Competence in :Surgical wound and pressure injury staging / treatmentInhalation therapyPhlebotomyBowel / bladder training programsCare / maintenance of catheters, drains, and durable medical equipmentAbility to prioritize, organize tasks, and function under stress.Maintains licensure, continuing education, professionalism, and confidentiality.Able to work independently and as part of a team; flexible schedule and local travel required.Required Qualifications
