Job Description :

We are seeking a highly skilled and empathetic Hospice Care Specialist to join our team. The ideal candidate will possess extensive experience in providing end-of-life care, demonstrating both clinical expertise and a deep understanding of the emotional and psychological needs of patients and their families.

Key Responsibilities :

• Deliver comprehensive hospice care to patients / clients in their homes.
• Provide emotional support to patients / client and their families.
• Administer medications and treatments as prescribed, ensuring patient comfort and symptom management.
• Conduct regular assessments and report to DON for adjustments to the care plans as needed to meet changing patient / client needs.
• Educate patients / clients and families about the hospice care process, addressing any questions or concerns.
• Maintain accurate and detailed patient records in compliance with regulatory standards.

Qualifications :

Licensed Practical Nurse (LPN) license.

Minimum of 3 years of experience in hospice or palliative care.

Strong clinical skills and the ability to manage complex medical conditions.

Excellent communication and interpersonal skills.

Empathetic and compassionate nature with a strong commitment to patient-centered care.

Ability to work independently and as part of a collaborative team.

Proficiency in electronic medical records (EMR) systems.
