Home Health Physical Therapist (PT) – San Francisco, CA

Make a meaningful impact as a Physical Therapist (PT) providing one-on-one care in the comfort of patients’ homes throughout San Francisco, CA. This Home Health PT role offers a flexible Monday–Friday schedule, a small 5–10 mile travel radius, and the opportunity to deliver truly personalized, relationship-focused care with award-winning support behind you.

Position Highlights

  • Setting : Home Health
  • Schedule : Monday–Friday, daytime; flexible scheduling; weekends off; autonomy to stack visits
  • Location : San Francisco, CA (serving patients within a 5–10 mile territory radius)
  • Population Served : Adult and geriatric home-based patients
  • Compensation : $130,000–$190,000 per year (DOE)

Key Responsibilities

  • Evaluate patient needs and develop individualized home-based physical therapy plans.
  • Deliver evidence-based therapeutic interventions to restore mobility and reduce pain.
  • Track patient progress and adjust treatment plans to support functional improvement.
  • Educate patients and caregivers on exercises, safety, and mobility strategies.
  • Collaborate with an interdisciplinary care team to ensure quality outcomes.
  • Maintain accurate, timely documentation using user-friendly digital tools.
  • Promote patient independence and provide compassionate, supportive care.

    • Qualifications

  • Physical Therapist (PT) — active, unrestricted California PT license
  • Minimum of 1 year PT experience
  • Current AHA CPR certification
  • Valid Driver’s License and reliable transportation
  • Strong communication, time-management, and clinical reasoning skills
