Essential Functions :

• Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations.
• Implements and updates care plans to address patient's nursing needs.
• Educates and supports patients, family, and staff on nursing care and patient needs.
• Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements.
• Supervises Licensed Practical Nurses and Hospice Aides.

Requirements :

Certified Hospice and Palliative Nurse (CHPN), preferred.

2+ years' experience in hospice or palliative care but willing to train the right person.

Ability to collaborate within an interdisciplinary setting.

Knowledge and understanding of hospice philosophy and principles of death / dying.

Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements.

Travel required.
