Position Requirement

• Under general supervision, the incumbent will plan for client services, provide basic services to individuals, and maintain records pertaining to individual progress. The incumbent must possess the knowledge, skills and abilities to fulfill the job requirements.
• A high school diploma or equivalent is required, associate degree preferred. Position also requires a valid driver license and acceptable driving record.
• Services to mentally ill and chemically dependent individuals is preferred. A flexible schedule may be required to meet the needs of the agency / programs.

DESCRIPTION :

The incumbent is responsible for assisting the case management department in providing case management services to individuals with mental illness(es) and / or substance use disorders. Emphasis will be placed on assisting the case management department with delivery of services for the SMI, SED, and ATR populations. This includes assisting the case management department with discharge planning, linking with community resources, outreach and follow-up. This position is directly responsible to the Senior Case Manager and Coordinator of Clinical Case Management Services. The position is exempt for FLSA. The position is designated as a Level Two with regard to the HIPAA Privacy Rule. OSHA regulations apply to this position.
