The Registered Nurse position is a very heavily community-focused position with the overall aim of promoting community wellness through health promotion, education, screening, and licensed independent practitioner-supervised intervention activities. The Registered Nurse provides general education in the pertinent areas of primary care and preventive care by utilizing standard nursing concepts and skills. Primary areas of focus include cancer prevention, women's health, reproductive health and sexual health and chronic disease management.
Essential Functions
- Offers education, and counseling to individuals and populations in a community health setting, through various events including general health fairs, focused topic events/activations and care through mobile health units.
- Serve as an integrated care team member by providing clinical advice, information, education, and counseling to clients for testing, diagnosing, and treatment of outpatient, community health diagnoses and illnesses, including sexually transmitted infection or blood-borne infections,
- Collaborates with City of Houston and/or Harris County Public Health for reporting and surveillance purposes of mandatory reportable illnesses and diseases.
- Perform age-based and risk-assessed screenings and administer immunizations and injections as appropriate
- Provide services to patients of all ages in an appropriate, culturally safe, and judgment-free environment
- Participates in El Centro's compliance, risk, and patient safety, quality assurance, and performance improvement (QAPI) activities.
- Perform supportive integrated care team tasks such as pre-visit planning, medication reconciliation, medication refills and telephone triage
- Bilingual in English/Spanish required (Written/Spoken)
- An active, unrestricted Texas RN license
- At least two years of paid experience in a community health setting
- Certified in CPR and AED
- Current Basic Life Support (BLS) Certification is required.
