We are seeking a Pharmacist to perform all long-term care pharmacy serving skilled nursing, senior living communities and discharge meds. This role ensures safe, accurate, and timely medication dispensing, regulatory compliance, and effective collaboration with long-term care partners.
Key Responsibilities:
• Verify, dispense, and oversee prescription medications in compliance with physician orders and all applicable federal, state, and Virginia Board of Pharmacy regulations.
• Review medication orders for accuracy, dosing, interactions, and therapeutic appropriateness; provide clinical recommendations and drug therapy guidance.
• Communicate with nursing staff and long-term care facilities to support medication management, interventions, and timely delivery.
• Supervise, train, and support pharmacy technicians and delivery personnel to ensure operational efficiency and compliance.
• Manage inventory for controlled and non-controlled medications, including required audits and documentation.
• Oversee order processing, refills, compounding (including sterile and IV preparations), insurance adjudication, and billing issue resolution.
• Maintain accurate records, protect patient confidentiality and e-PHI, meet continuing education requirements, and fulfill the physical demands of the role, including standing, lifting, and repetitive tasks.
Qualifications & Requirements:
• PharmD or BS in Pharmacy from an accredited school
• Active pharmacist license in Virginia, in good standing
