FULL TIME

Qualifications for Social Services:

• Bachelor’s Degree required;

• LSW licensure required; LCSW or LICSW

• A minimum 2 years’ experience working in a SNF with short

term rehab/discharge planning experience

• Excellent communication, documentation, and assessment skills

• Possess a genuine concern for and interest in people

Responsibilities for Social Services:

• Build relationships between residents and staff to support resident’s individual needs

• Find options that most meet the physical and emotional needs of each resident

• Maintain contact with family to report changes in health, current goals, and care planning

• Make referrals and obtain services from outside entities

• Provide or arrange needed counseling services

• Assist residents with financial and legal matters

• Review facility policies and procedures as part of the facility's interdisciplinary team to assure compliance with state and federal regulations
