Position Purpose : Under the supervision of the Clinical Manager, the Intake Coordinator is an essential member of the clinical team as a facilitator of clinical mental health assessments. The Intake Coordinator’s primary function is to facilitate the admissions process, which includes risk assessments and appropriateness for services (both therapy and psychiatry).

Core Duties and responsibilities, include but are not limited to :
Provide clinical assessment to determine appropriateness of services using screening questionnaires and risk assessmentsCoordinate the scheduling of intakes for therapists and psychiatry providersConduct record reviews (ROI, custody paperwork, etc.)Consult with Clinical Director, Manager, or Supervisor as neededReview internal referrals weekly, conduct outreach to referrals within 5 business days, document attempts, and close the loop by informing providers of the outcome of the referralAddress urgent referrals within 2 business days and close the loop by informing providers of the outcome of the referralCommunicate with the administrative team to ensure review of the patient’s insurance and eligibility coverage, and inform the patient of copay / good faith estimateProvide individual and group therapy, as neededEnsures all appointments are properly created, scheduled, and confirmedManage waitlist and waitlist outcomesAttend meetings as applicable to the job title. (i.e., all staff meetings)Behave professionally and work collaboratively with co-workers to increase productivityRefer therapy and psychiatry patients to outside specialists or agencies as needed, using clinical judgment and higher-level care parametersRespond to crisis situations and intervene therapeutically as necessary; assesses for the safety of the patient and assesses for suicidality, homicidal, and self-harm ideationComplete documentation within the organization policy within 72 hoursMaintain a capacity expectation of 60%Assist with quality measuresProvide clinical supervision to ASW and MSW internsPerform other duties and projects assigned by the Clinical Manager of Mental HealthThis job description in no way states or implies that these are the only duties to be performed by the employee. Education, Qualifications, and Experience :
LCSW / LMFT requiredBilingual : Spanish requiredMinimum 3 years of related experience2 years post licensure requiredExperience in clinical intake and assessment processesStrong computer skills, acute attention to detail, confident and professional communicationKnowledgeable in general EMR / EHR systems is preferredWork Schedule :
FTOC is an in-person organization first, and foremost. Employees are expected to be on-site for their scheduled shifts.Hours of operation are Monday to Friday 8 a.m. to 8 p.m., however, employee schedules vary, depending on organizational, staffing, community, and patient needs. As such, FTOC may need to modify work schedules to meet such needs.Holidays and weekends may be required depending on an employee’s department due to organizational, staffing, community, and patient needs as FTOC continues to grow and expand work days and hours.Overtime may also occur due to organizational, staffing, community, and patient needs.Work Location :
While employees may be assigned a primary site (Tustin, Garden Grove, Fountain Valley, Wellness Center, Mobile Units, Treehaven, Magnolia, Telehealth, etc.), employees may be required to temporarily fill in at another site based on organizational, staffing, community, and patient needs.Travel Requirements :
Occasional travel will be required. Employees must be able to travel to FTOC sites as needed, including with short notice.Physical Demands and Working Conditions :
