Are you a compassionate and skilled Registered Nurse eager to empower adults on their journey toward independence? If so, Aspire Health and Community Services invites you to join our dedicated Adult Community Clinical Services (ACCS) Team!

We are seeking a mission-driven RN who is eager to use their clinical skills to support individuals with mental health challenges, ensure safe and effective medical care, and play a vital role in a community-based program.

Responsibilities:

• Assess the community regularly to engage and provide resources to clients in their houses
• Assess health needs and advocate for essential medical services
• Collaborate with medical providers to implement treatment plans
• Ensure MAP compliance and complete regular medication audits
• Provide first aid, emergency responses, and direct nursing care
• Train, support, and consult with residential staff in residential group homes
• Participate in Recovery Planning Team meetings and interdisciplinary care

Requirements:

• Graduate of an accredited Nursing program (Diploma/ASN/BSN)
• Current RN license, CPR, and First Aid certifications
• Interdisciplinary collaboration experience
• Strong communication, documentation, and teamwork skills
• Valid driver's license and personal vehicle required for client transportation
• Experience working with individuals with mental illness, preferred.

Compensation is based on candidate experience, qualifications, and certifications and licenses. Annual ranges listed are based on a full-time, 40 hours per week position.

Salary Range

$83,700-$100,500 USD
