The Care Manager MSW provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive / behavioral or legal barriers to care access.

• This is an onsite position, requiring daily attendance at the designated location.
• Candidates must reside within commutable distance of the following office location :

• Must be available to work weekends and holidays as scheduled, based on operational and departmental requirements.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby / on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
