SUMMARY :
Under the direction of the Director of Nursing, the Outpatient Registered Nurse (RN) is responsible for delivering comprehensive, trauma-informed, and integrated medical care to clients receiving services through SFRC's Certified Community Behavioral Health Clinics (CCBHC). This includes conducting nursing assessments, performing point-of-care (POC) testing, supporting the management of chronic conditions, and coordinating care with behavioral health and primary care providers.
As a member of the interdisciplinary team, the RN works collaboratively to promote whole-person care and uphold CCBHC standards for quality, access, and accountability. The RN ensures compliance with all applicable state and federal regulations, nursing practice standards, and SFRC policies and procedures.

Reports to the Director of Nursing and Program Director
DUTIES AND RESPONSIBILITIES :
- Conduct nursing assessments and evaluate health needs of clients with co-occurring behavioral and physical health conditions.
- Perform point-of-care testing (e.g., glucose, HbA1c, lipid panels, urinalysis, rapid infectious disease screens, drug screens) to support timely and evidence-based decision-making.
- Monitor chronic medical conditions and provide follow-up interventions in collaboration with the interdisciplinary care team.
- Coordinate medical care and referrals to primary care and specialty services, ensuring integration of behavioral and physical health care.
- Collaborate with therapists, case managers, peer support specialists, and other team members to maintain a therapeutic environment and ensure continuity of care.
- Participate in treatment planning and care coordination meetings, offering clinical insight to support whole-person care plans.
- Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR) in accordance with CCBHC standards and regulatory requirements.
- Provide health education and wellness counseling to clients and their families, promoting preventive care and improved health literacy.
- Support quality improvement initiatives, including data collection for CCBHC quality metrics and outcome measures.
- Remain informed of current practices in integrated care, behavioral health nursing, and trauma-informed approaches.
- Attend mandatory staff meetings, training sessions, and continuing education to maintain licensure and stay current on best practices.
- Maintain client confidentiality in accordance with HIPAA, 42 CFR Part 2, and SFRC's privacy and ethical policies.
- Perform duties within the New Mexico Board of Nursing Scope of Practice for RNs.
- Perform other related duties as assigned by the Director of Nursing or Program Director.

SPECIAL SKILLS :
Strong assessment and clinical judgment skills with an emphasis on behavioral health and chronic disease management.Proficiency with point-of-care testing procedures and interpretation of results.Excellent written and verbal communication skills for documentation, interdisciplinary communication, and client engagement.Technologically proficient, including use of Electronic Health Records and Microsoft Office Suite.Ability to work effectively within a team-oriented, collaborative care model.Composure and professionalism in a fast-paced outpatient environment.Cultural responsiveness and commitment to serving the diverse populations of New Mexico.MINIMUM QUALIFICATIONS :
Registered Nurse License.? Must be licensed at the time of hire, and continue licensure throughout employment.One-year experience working with clients with co-occurring mental health / substance abuse problems preferred.Must be computer literate in order to effectively maintain compliance with licensing and certification standards and requirements for use of computerized client assessments, data systems, etc.Must have and maintain a current, valid NM Driver's License.? Must also have and maintain a clean driving record, as well as auto insurance.Bi-lingual / Bi-cultural preferred (English / Spanish).
