[job_card.job_description]Supervise patient care and treatments, lead assessments, create personalized care plans, and address patient concerns to deliver exceptional care. Develop meaningful relationships with patients and their families, understanding their unique needs to support their recovery. Collaborate and communicate effectively with various departments to ensure comprehensive care and provide direct patient care when needed. Act as a resource for team members, aiding them with complex patient needs and inquiries. Celebrate the achievements and milestones of our patients along their recovery journey. Valid RN license as per state regulations. CPR certification required (ACLS preferred). CRRN certification is a plus. Minimum of one year of experience in a rehabilitation hospital setting is preferred.
