ADMISSIONS COORDINATOR

Position Scope:
The Admissions Coordinator works directly with the Director of Admissions, Assistant Regional Directors, and all departments within RHD to assure that appropriate individuals are admitted efficiently, and that all prospective individuals are assessed and evaluated for the most appropriate level of care available. The Admissions Coordinator is involved in communicating and guiding potential individuals and families through all phases of the admissions process including, but not limited to, answering inbound calls and emails as well as making outbound calls to prospective individuals, their families and referral sources. To deliver program information, collect/document necessary information from the prospect to appropriately place the individual in the program that best supports the individual’s needs. This role is vital for filling vacancies and works in conjunction with the other admissions coordinators and all other necessary departments to facilitate seamless and timely admission to all RHD programs.
DIRECT DUTIES:
Systems, Workflows and Process Improvement Management
· Conduct thorough evaluations of referrals and compile detailed summaries to facilitate informed placement decisions.
· Respond promptly and professionally to inquiry calls and emails, ensuring that queries are handled efficiently.
· Coordinate closely with various referral sources to ensure seamless communication and understanding.
· Actively participate in strategic planning meetings with Office of Developmental Programs (OPD), Administrative Entities (AEs), and other key referral sources to collect essential information for the referral review process.
· Carry out comprehensive pre-admission assessments, including the review of clinical information for prospective individuals, while sharing findings with the referral and program teams to promote informed decision-making.
· Initiate proactive contact to collect vital clinical and demographic data from individuals and relevant sources to enhance the referral process.
· Communicate all pertinent information to the appropriate internal staff in a timely manner during the pre-admission process, fostering collaboration and clarity.
· Address inquiries from individuals, families, and referring providers professionally and promptly, ensuring any necessary information is relayed to the appropriate administrative, clinical, and medical staff when required.
· Maintain compliance with all regulatory requirements and ensure the detailed upkeep of additional documentation related to the admissions process.
· Offer assistance to individuals and their families throughout the admission process, making the experience as smooth and supportive as possible.
· Maintain consistent and accurate records using the Electronic Health Record (EHR) and other internal systems, ensuring data integrity and availability.
· Continuously update and maintain precise tracking of referral data, encompassing individuals’ demographics, referral disposition, and referral/admission outcomes to facilitate ongoing assessments and improvements.
· Strive for operational excellence, ensuring high call quality and exceptional client satisfaction at every touchpoint.
· Actively advocate for RHD to prospective individuals, families, and employees, highlighting the organization’s values and commitment to quality care.

· Supports/assists program and regional leadership in addressing issues and concerns with the admission process.
Departmental Collaboration
· Coordinates and reviews documentation and reports for external entities, such as Shared Service departments and the Service Lines.
· Supports the supervision of systems and processes for the Service Lines, Human Resources, and Training and Development.
· Supports/leads program and divisional operational activities, including special projects and acts as chair for referral committee.
· All other related duties as assigned.
Job Qualifications
Education and Experience:
· Bachelor's Degree in business administration or related field, preferred or
· Equivalent education and work experience
· Minimum 2 years of direct admissions or related field experience
