Overview We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensation-without on-call requirements.
Key Responsibilities: - Provide physician-ordered physical therapy services in patients' homes
- Evaluate patient function and develop individualized plans of care
- Complete initial evaluations, reassessments, and plan-of-care updates every 30 days
- Supervise Physical Therapy Assistants and Home Health Aides as appropriate
- Educate patients and caregivers on exercises and home care routines
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation, including OASIS
- Participate in quality initiatives, in-services, and care conferences

Requirements: - Bachelor's, Master's, or Doctorate degree in Physical Therapy
- Active Physical Therapy license in the state
- Valid driver's license and reliable transportation
- Must be a U.S. citizen or Green Card holder (no exceptions)
