Hiring : Speech-Language Pathologist (SLP)

Locations :

  • Chase Center Rehabilitation and Nursing Services - Logansport, IN
  • Warsaw Meadows Care Center - Warsaw, IN

Flexible Schedules Available :

  • Full-Time (32-40 hours / week) at Chase Center
  • Float Full-Time (32-40 hours / week) between Chase (2 days) and Warsaw Meadows (3 days)
  • Consistent PRN at both facilities : 3 days / week, 4-5 hours / day (15 hours / week per facility)

    • Chase Center Rehabilitation and Nursing Services, together with Warsaw Meadows Care Center, is seeking a caring and motivated Speech-Language Pathologist to join our in-house therapy team. With multiple scheduling options, you can build a role that best fits your lifestyle while helping residents improve their communication, cognition, and swallowing abilities.

    Responsibilities :

  • Provide skilled speech-language pathology services in line with professional standards and facility protocols
  • Conduct evaluations and develop individualized treatment plans with measurable goals
  • Deliver therapy targeting speech, language, cognitive-linguistic, voice, fluency, and swallowing disorders
  • Educate and train patients, families, and staff to promote carryover and maximize outcomes
  • Complete accurate and timely documentation in compliance with state and federal guidelines
  • Collaborate with the interdisciplinary team to support residents' overall care plans

    • Qualifications :

  • Licensed Speech-Language Pathologist in the state of Indiana
  • Master's Degree in Speech-Language Pathology from an accredited program
  • Must be at least 18 years of age
  • SNF or long-term care experience preferred, but not required
  • CFYs welcome to apply (depending on supervision availability)
