POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS - FL/compact RN license

BLS

2 years case management experience in an acute hospital setting

Acute hospital experience during the past 12 months with no more than a 90-day break

Prior experience in case management at an HCA facility

Prior experience with Meditech and Midas programs

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 8:00AM - 5:00PM

SYSTEMS - Meditech / Midas

START DATE - ASAP
