Essential Functions:

Administers the physical therapy treatment plan as developed by the Physical Therapist.
Community/Resident Engagement Activities (Fall/Risk Assessments, Walking Club, Education/Exercises, Building Relationship with Community Stakeholders (Executive Directors, Activity Directors, etc.)
Apply concepts of infection control and universal precautions in performing patient care activities.
Accepts clinical assignments that are consistent with education and competence to care for patients.
Uses effective interpersonal relations and communication skills.
Meets mandatory continuing education requirements.
Promotes compliance with all fiscal intermediaries and/or other third-party payers, though education, coaching, and other assistance as necessary.
Demonstrates commitment, professional growth, and competency.

Qualifications

Minimum of 1 years' experience in Home Health.
Homecare Homebase a plus.
Part B experience a plus.
