Job Title: Mobile Primary Care Physician – Aging Adult Population
Company: Twin Cities Physicians
Compensation: $225,000 – $250,000 annually
Location: Greater Twin Cities Metro Area

Position Summary:
We’re seeking a Physician (MD/DO) who is energized by the idea of working in diverse senior settings—from assisted living to memory care—and building long-standing trust with residents. If you're looking for a flexible, mobile, and mission-driven care role, this is it.
Key Responsibilities:
- Round at assigned senior living communities to deliver primary care services
- Provide medical oversight, diagnostics, and treatment planning
- Engage in chronic care management and reduce unnecessary ER visits
- Act as a clinical leader within an interdisciplinary support team
- Ensure thorough documentation and follow-up through EMR
Qualifications:
- Active Minnesota MD or DO license
- Experience in geriatric medicine, internal medicine, or primary care
- Ability to work independently while collaborating with facility teams
- Valid driver’s license and reliable transportation
