Position Summary

This Social Worker as a member of the Interdisciplinary Group (IDG), provides medical social services and support to patients and their families. The social worker will provide all psychosocial interventions and referrals, assessments, supportive counseling, networking, and resource procurement. Job duties are performed with awareness and respect of the patient's developmental stage, patient rights, and patient and family personal, cultural, and spiritual beliefs, according to specific program guidelines. Contributes to the smooth functioning of services, and advocates for patient / family needs with the team and community at large.

Essential Functions

• Conduct psychosocial assessments addressing emotional, mental health, and environmental needs.
• Provide therapeutic services including short-term counseling, crisis intervention, and bereavement support.
• Assist with advance directives, funeral planning, and resource referrals.
• Identify and coordinate access to internal and external resources.
• Participate in Interdisciplinary Group (IDG) meetings and contribute to care planning.
• Advocate for patient / family rights and support self-determination.
• Evaluate response to psychosocial interventions and document outcomes.
• Educate patients, families, and caregivers on psychosocial topics and coping strategies.
• Maintain timely and accurate documentation according to policy.
• Adhere to ethical and professional standards per NASW guidelines.
• Participate in on-call rotation and respond to urgent needs as required.
• Attend and contribute to internal and community-based meetings or trainings.
• Maintain confidentiality and comply with HIPAA regulations.
• Perform other duties as assigned within the scope of practice.
