The Norfolk Health District is seeking three () contract Community Health Workers (CHWs) to support public health initiatives in neighborhoods disproportionately impacted by the social determinants of health. CHWs will advocate for residents through local initiatives, engage with NDPH staff, and participate in community outreach. These positions are funded under the City of Norfolk's FY Safer Communities Grant Program and will focus on addressing the root causes of violence through health and social services.
Responsibilities :
Minimum Qualifications :
High school diploma / GED; strong communication and facilitation skills; community outreach experience; basic computer skills; ability to work independently; ability to lift – lbs; valid driver's license and reliable transportation; availability for evenings, nights, and weekends.
Preferred Qualifications :
Experience working with vulnerable populations; CHW Certification.
Additional Requirements :
Must be prepared for public health emergency response; must complete required incident management courses; must protect PHI and comply with OSHA requirements.
