We are privately owned, CHAP Accredited Home Health Agency based in Campbell, California. We currently provide home health care to patients in Santa Clara, San Mateo, San Benito county, Alameda County and Solano County with plans to expand our service area.

Our founders include highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality of clinical outcome and patient satisfaction.

Compensation: $50-$60 per hour DOE in home healthcare setting

Ideally this candidate will cover Santa Clara County OR San Benito County. If you are available to provide care in other areas, please let us know!

Responsibilities:

- Assist the Physical Therapist in providing treatment to patients with various conditions and injuries

- Implement therapeutic exercises and modalities under the direction of the Physical Therapist

- Monitor and document patient progress during treatment sessions

- Educate patients on proper techniques for exercises and activities- Assist with patient transfers and mobility as needed

- Maintain a clean and organized treatment area

Qualifications:

- Associate's degree from an accredited Physical Therapist Assistant program

- Valid state licensure or certification as a Physical Therapist Assistant

- Strong communication and interpersonal skills

- Ability to work effectively as part of a healthcare team

- Home health experience preferred, but not required

- Familiar with Wellsky/Kinnser EMR software a plus
